Donor Medallion

LifeLink invites you to participate in our donor family quilt project, dedicated to the memory of your loved ones who so generously gave the gift of life. Each quilt square commemorates their love and their life. The Quilt will be displayed annually at the Donor Remembrance Service and other special events to help spread the message of organ and tissue donation, and to remember those who gave.

Donor Family Quilt

Quilt patches should be 8" square, including a half inch margin (see pattern below) - and may be any color or fabric. Use crayons, paint, markers, thread or any other medium to design the square. Please feel free to include the name of your loved one and dates of birth and death, if you wish.

Please be sure to include your name, address and phone number with the square. This information will be kept confidential. We would like to provide you with a confirmation of the receipt of your quilt square. If you do not receive confirmation in 6-8 weeks, please notify the Public Relations Department at (800) 544-6667.

Feedback from donor families is sought through surveys and communication with their transplant coordinators. We continue to look for ways to improve our service to the families, who are the most important component of our organization.
